{"id":19137,"date":"2026-07-24T10:03:00","date_gmt":"2026-07-24T10:03:00","guid":{"rendered":"https:\/\/www.simpleonlinepharmacy.co.uk\/health-advice\/?p=19137"},"modified":"2026-07-24T10:03:01","modified_gmt":"2026-07-24T10:03:01","slug":"who-can-take-an-oral-glp-1","status":"publish","type":"post","link":"https:\/\/www.simpleonlinepharmacy.co.uk\/health-advice\/weight-loss\/who-can-take-an-oral-glp-1\/","title":{"rendered":"Who Can Take an Oral GLP-1?"},"content":{"rendered":"\n<h2 id=\"h-what-are-the-bmi-requirements-for-oral-glp-1\" class=\"wp-block-heading\">What are the BMI requirements for oral GLP-1?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Three BMI bands can make you eligible to take an <a href=\"https:\/\/www.simpleonlinepharmacy.co.uk\/health-advice\/weight-loss\/oral-glp-1-weight-loss\/\" type=\"link\" id=\"https:\/\/www.simpleonlinepharmacy.co.uk\/health-advice\/weight-loss\/oral-glp-1-weight-loss\/\">oral GLP-1 for weight loss<\/a>. This depends on your health and ethnicity. The Medicines and Healthcare products Regulatory Agency (MHRA) set these limits in June 2026. It approved oral semaglutide to treat obesity in adults.<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><tbody><tr><td><strong>BMI threshold<\/strong><\/td><td><strong>What it means<\/strong><\/td><\/tr><tr><td>30 or above<\/td><td>You can usually take an oral GLP-1 for weight loss on BMI alone, with no other condition needed.<\/td><\/tr><tr><td>27 to 30, with a weight-related condition<\/td><td>A qualifying condition brings you into range. Type 2 diabetes, high blood pressure, high cholesterol, prediabetes and obstructive sleep apnoea all count.<\/td><\/tr><tr><td>27.5 or above, for certain ethnic groups<\/td><td><a href=\"https:\/\/www.nice.org.uk\/guidance\/ng246\" target=\"_blank\" rel=\"noreferrer noopener nofollow\" rel=\"dofollow\" >Lower treatment thresholds apply<\/a> to people from Asian, Middle Eastern, Black African or African-Caribbean backgrounds. Here, the same health risks tend to appear at a lower BMI, so a figure around 27.5 is used instead of 30.<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><br>Each pharmacy has its own internal protocols, which allow prescribing under set guidance which may differ slightly from the above. Please make sure you provide all information honestly and accurately, so the prescriber has everything they need to assess whether the treatment you\u2019ve ordered is suitable for you.<\/p>\n\n\n\n<h2 id=\"h-who-should-not-take-an-oral-glp-1\" class=\"wp-block-heading\">Who should not take an oral GLP-1?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Like any medicine, oral GLP-1s are not suitable for everyone.<\/strong> You should not take an oral GLP-1 if you are pregnant, breastfeeding, trying to conceive, or allergic to semaglutide. Severe kidney or liver problems may also mean oral semaglutide isn\u2019t suitable for you.&nbsp;<\/p>\n\n\n\n<h3 id=\"h-pregnancy-and-conception\" class=\"wp-block-heading\">Pregnancy and conception<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Semaglutide can affect a developing baby. The <a href=\"https:\/\/www.medicines.org.uk\/emc\/product\/102347\/smpc\" target=\"_blank\" rel=\"noreferrer noopener nofollow\" rel=\"dofollow\" >product information<\/a> states to avoid oral Wegovy in pregnancy. If you\u2019re planning a pregnancy, we advise stopping treatment at least 2 months before trying to conceive to allow the medication to fully clear from your system. Please use reliable contraception while taking the Wegovy pill.<\/p>\n\n\n\n<h3 id=\"h-thyroid-history\" class=\"wp-block-heading\">Thyroid history<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The US<a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2025\/218316Orig1s000lbl.pdf\"> Food and Drug Administration (FDA)<\/a> warns against semaglutide use for anyone with a personal or family history of medullary thyroid carcinoma (MTC), a rare thyroid cancer. The same applies to an inherited condition called multiple endocrine neoplasia type 2 (MEN2). This traces back to studies in rodents, where GLP-1 medicines were linked to thyroid tumours. However, <a href=\"https:\/\/academic.oup.com\/endo\/article\/151\/4\/1473\/2456651\" target=\"_blank\" rel=\"noreferrer noopener\">large human trials<\/a> have not shown a thyroid cancer link.\u00a0<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The UK licence does not include this warning. We still ask about thyroid cancer in your family, though, so our prescribers can weigh up any risk.<\/p>\n\n\n\n<h3 id=\"h-gallbladder-problems\" class=\"wp-block-heading\">Gallbladder problems<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Gallbladder problems are worth a mention because losing weight quickly can raise the risk of developing gallstones. In the <a href=\"https:\/\/www.nejm.org\/doi\/10.1056\/NEJMoa2500969\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Wegovy tablet trial<\/a>, acute gallbladder disease (which included gallstones) affected around 1 in 40 people. The rate was a little higher than in the placebo group, where patients were given an inactive pill for comparison purposes.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Having your gallbladder removed doesn\u2019t automatically rule out oral GLP\u20111 treatment, and many people without a gallbladder take these medicines safely. Please let us know if you\u2019ve had any gallbladder problems in the past, so we can assess your suitability.<\/p>\n\n\n\n<h3 id=\"h-inflammatory-bowel-disease\" class=\"wp-block-heading\">Inflammatory bowel disease<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Let us know if you have a serious bowel condition, such as Crohn&#8217;s disease or ulcerative colitis. Treatment commonly causes gut side effects. These can hit harder if your bowel is already inflamed or easily irritated.<\/p>\n\n\n\n<h3 id=\"h-type-2-diabetes\" class=\"wp-block-heading\">Type 2 diabetes<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Having type 2 diabetes doesn\u2019t rule treatment out, but it does mean you\u2019ll need closer monitoring. Tell both your diabetes team and us before you start.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Rybelsus is a lower-dose oral semaglutide already prescribed to treat type 2 diabetes. Having the condition does not rule out your taking an oral GLP-1.<\/p>\n\n\n\n<h2 id=\"h-are-there-any-medicines-that-interact-with-oral-glp-1s\" class=\"wp-block-heading\">Are there any medicines that interact with oral GLP-1s?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Yes, oral GLP-1s interact with several common medicines.<\/strong> The main groups include diabetes treatments, thyroid tablets and blood thinners.You should never take an oral GLP\u20111 alongside another <strong>GLP\u20111 medicine<\/strong>.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Semaglutide reduces blood sugar levels as well as your appetite. Taken with insulin or certain diabetes tablets, it can push your blood sugar too low. Doses of your other medications may need adjusting to compensate and keep you safe.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Medicines like levothyroxine and warfarin can usually continue, but both need closer monitoring once an oral GLP-1 is added.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Wegovy pill drug interactions are broadly similar to those seen with other GLP\u20111 medicines, whether injectable or oral. However, with <strong>ora<\/strong>l semaglutide, dose timing matters. Take your Wegovy tablet on an empty stomach after an 8 hour fast with up to 120mls of water.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Food, drink and other medications can significantly <a href=\"https:\/\/link.springer.com\/article\/10.1007\/s13300-021-01078-y\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">lower how much medication your body absorbs.<\/a> Wait at least 30 minutes before taking anything else.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Before approving treatment, our prescribers review your full medical history. This includes checking any current medicines against the known Wegovy and Foundayo drug interactions. Tell us about everything you take: prescription or bought over the counter medicines, vitamins, supplements and herbal formulations. This helps us assess whether a GLP-1 weight loss pill is right for you.<\/p>\n\n\n\n<h2 id=\"h-how-do-i-get-an-oral-glp-1-on-prescription-in-the-uk\" class=\"wp-block-heading\">How do I get an oral GLP-1 on prescription in the UK?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>You are able to get an oral GLP-1 in the UK via private prescription, because the NHS does not yet fund any.<\/strong> Complete a consultation about your health, your weight and your medical history. A qualified\/registered prescriber or other healthcare professional then reviews your answers and decides whether the treatment suits you.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Oral GLP-1 eligibility is a clinical decision, not an automatic one. Our prescribers review your BMI, medical history and the medicines you already take. We also consider any past weight loss treatments and whether the daily dosing routine is practical for you.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Right now, oral Wegovy is the only oral GLP-1 licensed for weight loss in the UK.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Foundayo (orforglipron), the tablet from Eli Lilly, was US-approved in April 2026, but the MHRA has not yet approved it here.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">More oral GLP-1 tablets should follow as they pass UK review. Oral semaglutide is the one our service can prescribe today.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you already use injectable semaglutide, <a href=\"https:\/\/www.simpleonlinepharmacy.co.uk\/health-advice\/weight-loss\/wegovy-pill\/switching-injection-to-oral-wegovy\/\">you may be able to switch to the 25 mg tablet<\/a>. This applies if you are currently taking the 2.4 mg maintenance dose. You will start one week after your last injection. Weight management is a long-term commitment, and you can step up a dose only after at least 30 days at your current strength.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Wegovy pill helps you lose weight alongside diet and increased physical activity. <a href=\"https:\/\/www.nejm.org\/doi\/10.1056\/NEJMoa2500969\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">In the OASIS 4 clinical trial<\/a>, the 25 mg dose led to around 13.6% of body weight lost over about 15 months. Your own result may land higher or lower. It still gives a useful benchmark before you start.&nbsp;<\/p>\n\n\n\n<h2 id=\"h-when-should-i-see-a-doctor\" class=\"wp-block-heading\">When should I see a doctor?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Stop taking oral semaglutide and call 999 or go to A&amp;E if you notice any of these:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Severe stomach pain that spreads to your back, which can be a sign of pancreatitis. You may also experience vomiting, nausea and fever.&nbsp;<\/li>\n\n\n\n<li>Sudden loss of vision or any other vision changes<\/li>\n\n\n\n<li>A serious allergic reaction, with swelling of the face or throat and trouble breathing<\/li>\n\n\n\n<li>Signs of severe dehydration:\n<ul class=\"wp-block-list\">\n<li>feeling dizzy,&nbsp;<\/li>\n\n\n\n<li>passing little or no urine,<\/li>\n\n\n\n<li>being too weak to keep down any fluids<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Reviews have <a href=\"https:\/\/www.gov.uk\/government\/news\/mhra-finds-evidence-does-not-support-a-link-between-glucagon-like-peptide-1-glp-1-receptor-agonists-and-suicidal-and-self-injurious-thoughts-and-act\" target=\"_blank\" rel=\"noreferrer noopener nofollow\" rel=\"dofollow\" >not established a link<\/a> between oral GLP-1s and changes in mental health. But if you are having thoughts of harming yourself, do not wait. Call 999 or go to A&amp;E if you feel unsafe, and ask for the CRISIS team. You can also call the Samaritans free on 116 123, at any time of day or night.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For most people, side effects are mild. They are commonly gastrointestinal side effects: nausea, vomiting, diarrhoea and constipation. These usually ease within the first few weeks of starting treatment or moving up a dose.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nausea and vomiting are the side effects to expect early, affecting more than 1 in 10 people. If they are wearing you down, or you are not sure whether to stick to your your dose, contact our clinical team.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Some symptoms need faster attention. See your GP or call <a href=\"tel:111\">NHS 111<\/a> if vomiting or diarrhoea stops you keeping fluids down for more than a few hours. It could tip into dehydration.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Yellowing of your skin or the whites of your eyes needs same-day advice. So does pain in the upper right of your stomach. These symptoms may point to a gallbladder problem.&nbsp;<\/p>\n\n\n\n<h2 id=\"h-frequently-asked-questions\" class=\"wp-block-heading\">Frequently asked questions<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Am I eligible for an oral GLP-1?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Most likely, yes, if your BMI is 30 or above. A BMI of 27 or above can also qualify you, as long as you have a weight-related condition. Before approving anything, our prescribers look at your medical history, the medicines you take and any weight-loss treatments you have tried before.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Can I take an oral GLP-1 with type 2 diabetes?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Often, yes. For some people, it works on both fronts. It eases weight while helping to regulate blood sugar. The active ingredient is already familiar in diabetes care.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A lower-dose oral semaglutide, called Rybelsus, treats type 2 diabetes. The weight-loss dose sits higher, so we check how it fits alongside your current diabetes treatment before you start. As we only prescribe semaglutide for weight loss, we will not prescribe Rybelsus for type 2 diabetes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Can I get an oral GLP-1 on the NHS?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Not yet. The NHS does not currently fund oral semaglutide for weight loss, so the route to it is a private prescription. Wider access could follow if the <a href=\"https:\/\/www.nice.org.uk\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\" rel=\"dofollow\" >National Institute for Health and Care Excellence (NICE)<\/a> recommends it for NHS use.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>What if my BMI is below 27?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">No oral GLP-1 carries a weight-loss licence below a BMI of 27, even with a weight-related condition. In some cases, it can be prescribed, although we will need to check your waist circumference as part of your assessment.<br><br>Your GP is the best next step if weight is affecting your health.
