What is Estring?

Estring vaginal delivery system is a vaginal ring that is inserted into the vagina and releases oestrogen. It is a type of Hormone Replacement Therapy (HRT) used to relieve vaginal symptoms of the menopause. It does not relieve other symptoms of the menopause such as hot flushes or night sweats. 

How does it work?

Oestrogen levels in the body decline around the time of the menopause. This can cause various symptoms associated with the menopause such as hot flushes, reduced libido, night sweats, and difficulty sleeping. 

This drop in oestrogen can also cause vaginal symptoms such as:

  • Vaginal dryness before/during sex
  • Vaginal discomfort
  • Recurring or frequent vaginal or urinary infections
  • Itching, irritation or burning
  • Pain during sexual intercourse

Decreased levels of oestrogen in the vagina can cause thinning of the vaginal walls, causing the above symptoms. It is known as vaginal atrophy.

Estring vaginal ring contains the active ingredient estradiol (the female hormone oestrogen) which is constantly released at a gradual rate. This oestrogen is released locally in the vagina and helps to maintain adequate levels of oestrogen which helps to relieve the vaginal symptoms listed above. 

Each ring contains enough oestrogen for 90 days. After 90 days it should be removed and replaced by a new ring. The maximum recommended length of continuous treatment is 2 years.

Inserting the ring

To insert the ring into your vagina:

  • Find a comfortable position
  • With one hand, open the folds of skin around the vagina
  • With the other hand, squeeze the ring into an oval shape
  • Push the ring into your vagina as far as it will go - upwards and backwards towards the small of the back

You may be able to feel the ring at first but this feeling should subside over time.

To remove the ring:

  • Choose a comfortable position
  • Place a finger into your vagina and hook it around the ring
  • Pull the ring gently out - downwards and forwards

Alternatives

Estring is known as an intravaginal oestrogen because it works locally in the vagina. Other intravaginal oestrogen products available are a cream (Ovestin), gel (Blissel), and vaginal tablets (Vagifem).

How do you insert Estring?

To insert the ring into your vagina:

  • Find a comfortable position
  • With one hand, open the folds of skin around the vagina
  • With the other hand, squeeze the ring into an oval shape
  • Push the ring into your vagina as far as it will go - upwards and backwards towards the small of the back

How long does it take for Estring to work?

It might take several weeks for the ring to have a full effect. As the ring begins to work you may notice an increase in vaginal lubrication.

Does Estring cause weight gain?

Other forms of oestrogen containing HRT can cause weight changes as a side effect. Estring may also cause this side effect but it is less likely compared to oral HRT medication.

Side Effects

There is always a risk of side effects when taking any medicine.

Commonly reported side effects of Estring include:

  • Urinary tract infection
  • Pain on passing urine
  • Itching
  • Increased sweating
  • Abdominal pain or discomfort
  • Infection and itching inside and around the vagina 

HRT can be very beneficial for women with menopause symptoms however, there are risks associated with treatment. The following diseases are reported more often in women taking HRT compared to women not taking HRT:

  • Breast cancer
  • Blood clots (venous thromboembolism)
  • Heart disease
  • Stroke
  • Endometrial (womb) cancer 
  • Ovarian cancer

Treatment with Estring should be reviewed by your doctor at least annually. It is also a good idea to know some of the symptoms associated with these conditions. More information can be found on the NHS website for signs of a blood clot, breast cancer, endometrial cancer and stroke as well as information on the risks of HRT.

The above are risks of treatment with any HRT. However, because Estring releases oestrogen into the vagina, there is less oestrogen in the bloodstream compared to other HRT treatments like oral tablets. This means that the above risks are less likely with Estring than some other HRT treatments.

Taking oestrogen-only HRT can increase the risk of endometrial (womb) cancer. It is uncertain whether the risk is similar with Estring because only low amounts are absorbed into the bloodstream.

Interactions with other medicines

Due to low levels of oestrogen from Estring reaching the bloodstream, interactions with other medicines are less likely (compared to other HRT products). However, Estring may interact with the following:

  • Anti-epileptic medicines e.g phenytoin, carbamazepine
  • St John’s Wort (a herbal medication)
  • Anti-HIV medicines e.g efavirenz, ritonavir
  • Anti-infective medicines e.g rifabutin, rifampicin
