Can I switch from Mounjaro to the Wegovy pill?

Yes – The Medicines and Healthcare Regulatory Agency (MHRA) has approved the Wegovy pill on 11 June 2026. UK Mounjaro users now have the option of switching to the Wegovy pill.

Why do people switch from Mounjaro to the Wegovy pill?

There can various reason to switch from Mounjaro to the Wegovy pill such as convenience, side effects or pricing. Eli Lilly increased the UK list price of Mounjaro by up to 170% from 1 September 2025. Anyone on the higher Mounjaro doses privately saw monthly costs double overnight.

Mounjaro supply problems have previously been problematic, but these have eased through 2026. Affordability is now the main concern for most patients. Oral Wegovy does not need cold storage or an expensive pen device, so it costs less than Mounjaro. 

The SELECT trial gave injectable Wegovy to over 17,000 adults with heart disease for three and a half years. The trial suggests that the risk of heart attack, stroke, and death from heart disease in those who had already experienced these before dropped by roughly 20%.

NICE (the National Institute for Health and Care Excellence) subsequently recommended Wegovy injections for NHS prescribing in May 2026. Available for adults with a BMI of at least 27 who have had a heart attack, stroke, or poor circulation in the legs. The Wegovy pill may receive similar NHS approval at a later date, though this has not been confirmed. Switching from Mounjaro to Wegovy injections is an option if you want to benefit now.

Some patients simply prefer a daily pill when weighing up their weight loss options. Needle anxiety is a factor for many. For others, the real concern is fitting injections around travel and daily routine.

Will I lose less weight on the Wegovy pill than on Mounjaro?

You should expect slightly less weight loss on average. A Wegovy pill vs Mounjaro head-to-head trial has not yet been carried out, but SURMOUNT-5 compared the two weight loss injections directly. Over 750 adults with obesity took either Mounjaro or Wegovy for 72 weeks. Mounjaro users saw an average of 22.5% weight loss compared to around 17% with Wegovy injections.

The OASIS 4 trial followed 307 adults who were overweight or obese for 64 weeks. Weight loss results on the Wegovy 25 mg tablet averaged around 17%, similar to the injection. For many people, the heart benefit or lower cost outweighs a smaller weight loss number than with Mounjaro.

How long do I wait between stopping Mounjaro and starting the Wegovy pill?

We advise a one week gap between your last Mounjaro injection and your first Wegovy pill. You should start the pill on your next scheduled Mounajro injection day. Tirzepatide takes four to five weeks to fully leave your system. Starting oral Wegovy too soon risks overlapping the two medicines and their potential side effects.

What dose do I start the Wegovy pill at after Mounjaro? 

While Wegovy and Mounjaro operate differently, you can swap between them.

Wegovy pill dose escalation runs from 1.5 mg through 4 mg, 9 mg and a 25 mg maintenance dose. You spend at least 30 days at each step.

Depending on what strength of Mounjaro you have been using, you can start the Wegovy pill at a higher strength.

Wegovy tablets come with strict timing rules that will be new to anyone moving from weekly injections. You take the tablet first thing in the morning with a half glass of water (120 mL maximum). Then wait 30 minutes before eating, drinking, or taking other medication. Food reduces absorption significantly, and most patients find this harder to stick to than a weekly injection.

Will I gain weight during the transition?

Some patients may find Wegovy pills less effective than Mounjaro. Weight usually stabilises within a couple of months of reaching the 25 mg maintenance dose.

What side effects should I expect when switching?

Nausea, constipation, and diarrhoea are most common during dose escalation. Around three in four people in the OASIS 4 trial reported gut symptoms at some point, though most settled with time. The side effects of switching from Mounjaro to Wegovy are not always predictable. Tolerating one drug well does not guarantee tolerating the other.

Two safety updates are worth knowing about. In January 2026, the MHRA strengthened its pancreatitis warning for all GLP-1 medications, including Mounjaro and Wegovy. Any new severe stomach pain during a switch needs urgent attention. The MHRA also flagged NAION in February 2026, a rare eye condition linked to semaglutide that can cause sudden vision loss in one eye.

When should I see a doctor?

Not every symptom needs the same response. If your side effects are mild or you have questions about your starting dose, talk to our clinical team. We can adjust timing, slow the dose ladder, or suggest small changes that usually settle the gut symptoms within a week or two.
