How they work: semaglutide vs tirzepatide
How Wegovy pills compare to Mounjaro starts with the science. They work differently.
The Wegovy pill contains semaglutide, a type of medicine called a GLP-1 receptor agonist. Semaglutide copies a gut hormone your body makes naturally. That hormone, GLP-1, signals fullness to your brain and slows digestion.
Tirzepatide, the active ingredient in Mounjaro, is made by Eli Lilly. Unlike semaglutide, it targets two gut hormones: GLP-1 and GIP. GIP plays a role in appetite and blood sugar. Targeting both hormones is thought to be why Mounjaro helps people lose weight faster in trials.
Efficacy: what the evidence shows
Until SURMOUNT-5, nobody had tested Mounjaro vs Wegovy head-to-head. This trial ran for 72 weeks. 751 adults with obesity were given one of the two drugs by injection, at the highest dose they could tolerate. The Mounjaro group lost an average 20.2% of their body weight. The Wegovy group lost 13.7%.
So is Mounjaro more effective than Wegovy pills? The injectable data says yes. But SURMOUNT-5 compared two injectable treatments. Everyone in the Wegovy group received the injection at doses up to 2.4mg. Nobody in the trial took the pill. To this day, no trial has tested the Wegovy pill against Mounjaro. Any oral Wegovy vs Mounjaro comparison is based on separate studies.
Wegovy pills weight loss results show an average of 13.6% body weight lost in just over a year. This data comes from the OASIS 4 trial.
If your BMI is above 45, Mounjaro’s extra weight loss could take you below health risk levels that the Wegovy pill may not reach.
If your BMI is 30–35, the weight loss from oral Wegovy may already be enough to reach a healthy range. At that starting point, the extra weight loss changes the number on the scale, not your actual health risk.

What does each treatment actually ask of you?
The rules are strict. You take the Wegovy pill first thing in the morning, on an empty stomach, with no more than 4 ounces/120ml of plain water. Nothing else for 30 minutes. No food, no coffee, no other tablets. These may dilute the medication, affecting the absorption and effectiveness.
Why? The pill contains an absorption enhancer called SNAC. It only works properly on a completely empty stomach. Break these rules, and the drug may not get absorbed.
Mounjaro is different. One injection a week, at any time of day — no food rules.
None of that matters if you have a needle phobia. For some people, the injection itself is the problem. The Wegovy pill takes that problem away.
Side effects
Both treatments can upset the gut. Nausea, vomiting, diarrhoea, and constipation are all common side effects. That part is the same.
In OASIS 4, 74% of participants on oral semaglutide reported at least one gut-related side effect. In SURMOUNT-5, more people stopped treatment because of side effects on injectable semaglutide than on tirzepatide.
When comparing Wegovy pill vs Mounjaro side effects, symptoms are similar, but timing often differs. You take the Wegovy pill after fasting overnight, so nausea hits before you’ve eaten. The side effects of Wegovy pills are not severe for most people, but nausea on an empty stomach every morning becomes the reason some quit.
With Mounjaro, you can eat first. That makes a difference. Some people get itching or redness at the injection site, but it tends to be mild.
UK access
The MHRA approved the Wegovy pill in June 2026, making it the first oral GLP-1 licensed for weight management in the UK. Mounjaro and injectable Wegovy are also available through the NHS and private prescription.
You typically need a BMI of 30 or above, or 27 or above with a weight-related health condition like high blood pressure, type 2 diabetes or high cholesterol. Lower thresholds apply for people from South Asian, Chinese, Black African or Black Caribbean backgrounds. Your prescriber will review your medical history before you start your weight loss journey.

Which is right for you?
The Wegovy pills vs Mounjaro decision comes down to trade-offs.
Mounjaro suits you if maximum weight loss is your priority. If your starting BMI is high, the extra weight loss will help you to reach health benefits that the pill may not. Its weekly routine suits busy or unpredictable mornings.
Avoid it if needle phobia is a genuine barrier. No amount of trial data changes anything if you won’t inject.
The Wegovy pill suits you if the thought of injecting has stopped you from starting treatment. At a BMI of 30–35, the pill’s weight loss may be enough to reach a healthy range on its own.
Avoid it if your mornings are unpredictable or if you already get morning nausea. The 30-minute fast is not optional. If you can’t do it reliably, the pill won’t work.
The choice between Wegovy pills vs the injection comes down to which format fits your life. Your weight loss journey depends as much on the routine as on the drug. Either way, diet and exercise alongside the medication will get you further than the drug alone.

- Sudden changes to your vision. In 2026, the MHRA flagged a rare eye condition called NAION in people taking semaglutide.
- Vomiting or diarrhoea that won’t settle. This can lead to dehydration and kidney problems.
