What is the Foundayo results timeline?
Foundayo takes a few weeks to ease appetite, and most of its results land within the first six months.
In the ATTAIN-1 trial, the highest-dose group was compared against a placebo group. Placebo means an inactive pill given for comparison purposes. Weight was already lower than the placebo group at the first check, four weeks in. People lost roughly 5% of their body weight by 12 weeks, and around 9-10% by six months.
Weight loss continued after six months, but far more slowly, reaching about 11% by 72 weeks. Only the first month or so is genuinely quiet. That is the normal pattern rather than a sign the medicine has failed.
Foundayo (orforglipron) is not approved for use here yet. The UK’s medicines regulator, the MHRA, has not signed it off. MHRA is short for the Medicines and Healthcare products Regulatory Agency.
The US Food and Drug Administration approved Foundayo in April 2026. The Foundayo results timeline here comes from the trial data, not from UK prescribing.
| What you are tracking | Typical timing in the trial |
| Foundayo starts working in your body | From your first daily dose |
| Appetite and hunger start to ease | Over the first few weeks |
| Around 2% of body weight lost | By 4 weeks, on the 0.8 mg starting dose |
| Around 5% of body weight lost | By 12 weeks (about 3 months) |
| Reaching the top 17.2 mg dose | No sooner than about 5 months |
| Around 9-10% of body weight lost | By 6 months, with the fastest stretch behind you |
| Weight loss flattens out | From around 11 months |
| Peak weight loss, around 11% | Around 72 weeks (about 16 months) |
Most of the weight loss happens in the first six months.
When does Foundayo start working?
Foundayo starts to ease appetite within the first few weeks, though most people feel it as a slow change rather than a sudden switch. Nobody wakes up on day ten with their hunger switched off.
Orforglipron, the active ingredient in Foundayo, is an oral GLP-1 receptor agonist. It copies GLP-1, a gut hormone you already make after eating. That hormone has two jobs: telling your brain the meal is finished, and slowing the rate your stomach empties. Food then sits with you for longer than usual. Hunger loses its edge without disappearing altogether.
The trial did not test Foundayo on its own. Everyone taking part also received lifestyle support, including a target of 150 minutes of activity each week. Foundayo works best alongside a healthy, balanced diet and increased physical activity.
How fast Foundayo works on hunger varies from person to person. Hunger usually shifts before the scales do, and feeling full sooner at meals is often the first sign. The ATTAIN-1 trial did not measure appetite directly. The timing here reflects the general GLP-1 pattern rather than a trial finding. What the trial did record was weight, already lower than placebo at the first check, four weeks in.
How do the doses of Foundayo build up?
Foundayo climbs through six strengths, starting at 0.8 mg and rising no faster than one step every 30 days. The appetite effect takes time to reach full strength. This slow dose escalation is there for a reason. It gives your stomach time to adjust and keeps side effects down.
The point of the ladder is tolerability, not speed. The doses of Foundayo run from 0.8 mg, then 2.5 mg and 5.5 mg, up to 9 mg, 14.5 mg, or 17.2 mg once daily. Reaching the top dose takes at least about five months, and only if it is needed and well tolerated.
Foundayo is taken once daily orally, with or without food. Unlike oral semaglutide, it does not need an empty stomach, and it has no food or water restrictions.
What weight loss can you expect in the first 3 months?
Trial participants lost around 5% of their body weight by 12 weeks. It is around the three-month mark of your journey on Foundayo when you see results on the scales. For someone weighing 100 kg, 5% works out at about 5 kg.
The first four weeks are the quietest part. The same person starting at 100 kg would be around 2 kg down at this point. How long for Foundayo to work in full comes down to your dose. The full weight loss results then arrive faster than most people expect, well before reaching the top strength. Nearly half of the total lands in this opening stretch.
How much weight loss happens by 6 months?
By 6 months, people on the highest dose of Foundayo had lost around 9-10% of their body weight. That is most of what the medicine delivers.
ATTAIN-1 tracked weight month-by-month in adults with obesity or overweight. Those in the overweight group also had a weight-related condition. More than half of the highest-dose group lost at least 10% of their body weight. Around a third lost at least 15%.
The two lower doses did less than the top strength. People on 5.5 mg lost roughly 7% of their body weight, while those on 9 mg lost about 8%.
Orforglipron has also been studied in adults with type 2 diabetes. Average weight loss results tend to run a little lower.
What happens after 6 months?
Weight loss carries on after six months, but much more slowly. The average only crept up to about 11% by 72 weeks. Weight loss almost levelled off after around 11 months. That last stretch looks more like holding the result than adding to it. ATTAIN-1 stopped at 72 weeks. What happens beyond about 16 months has not been measured yet.

Do side effects affect how quickly Foundayo works?
Side effects do not tell you how well Foundayo is working, though stopping because of them will affect your results. Nausea leads the list, and it climbs with the dose. Around 35% of people on the top strength reported it, against 10% taking a placebo.
The other common side effects of Foundayo are constipation, diarrhoea, vomiting, and indigestion. Gastrointestinal symptoms reached around 70% at the top dose. Nearly 40% of those on placebo reported them, too. Not everything you feel in the early weeks belongs to the medicine.
Timing tracks the dose ladder rather than the calendar. Nausea, vomiting and diarrhoea were all more common while the dose was climbing, and they eased once it settled. Severe cases were very much the exception. Among people who did report gastrointestinal trouble, the majority were mild – only 4% rated it severe. Across everyone taking Foundayo, around 5% stopped because of stomach side effects.
When should I see a doctor?
Most Foundayo side effects are mild, but a few symptoms need prompt medical help. Before starting, one safety point matters most. Foundayo’s US label says people with a personal or family history of medullary thyroid cancer (MTC) should not take it. The same applies to multiple endocrine neoplasia syndrome type 2 (MEN 2). Orforglipron did not cause thyroid tumours in animal studies, but the warning stays on the label as a precaution.
Most of what you notice early is mild and manageable. Our clinical team can help with mild side effects and dose questions:
- Persistent but manageable nausea, vomiting, diarrhoea, or constipation
- Heartburn, reflux, or changes in appetite
- Questions about when to step up or stay on your dose
Some symptoms are less common but need same-day attention. Contact your GP or NHS 111 if you notice:
- A new lump in your neck, a hoarse voice, or trouble swallowing
- Yellowing of the skin or eyes, or severe pain in the upper-right stomach area
- Vomiting or diarrhoea so bad that you cannot keep fluids down
- New changes in your vision, especially if you have type 2 diabetes
- Signs of low blood sugar if you also take insulin or a sulfonylurea, such as shakiness, sweating, or confusion
A small number of symptoms are emergencies. Stop Foundayo and call 999 or go to A&E if you notice any of the following:
- Severe stomach pain that spreads to your back, which can signal pancreatitis
- A severe allergic reaction, such as swelling of the face, lips, or throat, or trouble breathing
- Signs of severe dehydration
- Thoughts of harming yourself (go to A&E and ask for the CRISIS team)
Common questions about the Foundayo timeline
How quickly does Foundayo work compared with the injections?
No trial has put Foundayo head-to-head against Mounjaro or Wegovy. Trials have not measured the speed of results side by side. Separate trials are hard to compare fairly. The people enrolled differ, and the trials were run years apart.
The one thing those trials do agree on is the size of the result, not its pace. Foundayo averaged around 11% at its top dose. Wegovy reached about 15% in its own trial, and Mounjaro about 21%. Some people are happy to accept a smaller result from a tablet over a bigger one from an injection.
How long until Foundayo works, and what if it feels slow at first?
Reaching the top dose of 17.2 mg takes at least five months. Most of the weight loss has already happened by then. Only the first month or so is genuinely slow, so try not to judge Foundayo in the first few weeks.
How long for Foundayo to work, and does Foundayo keep working once you reach the top dose?
Foundayo works over months rather than weeks. Most of the weight loss happens within the first six months, and the pace slows after that. Weight reduction continues after you reach the top dose of 17.2 mg, but much more slowly. The final six months of the trial added very little.
When does Foundayo kick in biologically?
Orforglipron starts acting from your first dose, but blood levels rise gradually as you step up the dose over the first few months.
Orforglipron: how long to work?
Orforglipron is simply the active ingredient in Foundayo, so the timeline is identical whichever name you search.
Key takeaways
- Foundayo works over months: appetite eases within weeks, and the scales follow through the first half-year.
- By 12 weeks in ATTAIN-1, people had lost around 5% of their body weight. This rose to about 11% by 72 weeks on the highest dose.
- Most of the weight loss happens in the first six months.
- Foundayo is a once-daily oral GLP-1 receptor agonist taken to support weight loss. It is not yet approved in the UK.
- Judge Foundayo over its first six months rather than its first few weeks.
