Can the Wegovy pill help with PCOS-related weight gain?
Yes, the Wegovy pill can ease the weight gain that PCOS tends to drive. The Wegovy pill and PCOS are worth looking at together. The medicine targets both weight and insulin resistance, the pair of problems at the heart of the condition. The connection between Wegovy pill interactions and conditions such as PCOS are still being studied.
Polycystic ovary syndrome (PCOS) is common. The World Health Organization estimates it affects roughly 1 in 10 women of reproductive age. You may also be more likely to have PCOS if a close relative has it or if you are from a South Asian background.
People with PCOS tend to see irregular or missing periods, increased hair on the face or body, acne, and weight gain. Insulin resistance is one of the main reasons that weight can be harder to shift. High insulin levels as a result of that resistance adds to the weight gain and can unsettle your periods.
The Wegovy pill is the tablet form of semaglutide, the same medicine used in the Wegovy injection. The Medicines and Healthcare products Regulatory Agency (MHRA) approved the oral weight loss treatment in June 2026.
Adults with obesity or who are overweight with a related health condition can qualify for treatment. The Wegovy pill works on appetite and blood sugar levels at the same time. For now, you can get it privately rather than on the NHS.

How does the Wegovy pill work for PCOS?
Semaglutide, the medicine in oral Wegovy, works by copying a hormone your gut releases after you eat. It belongs to a group of medicines called GLP-1 receptor agonists, short for glucagon-like peptide-1.
That hormone (GLP-1) tells your brain you are full, so the feeling of fullness comes sooner and lasts longer. It also slows how fast your stomach empties, which reduces spikes in your blood sugar after meals.
Insulin resistance is one of the core problems in PCOS. The body compensates by making extra insulin to keep blood sugar steady. That excess insulin can push the ovaries to make more testosterone.
Losing weight is the most reliable way to improve insulin sensitivity in PCOS. Semaglutide may add a little direct benefit on top, though the weight loss itself does most of the work. Being a medicine that helps to reduce weight and improves insulin resistance, oral Wegovy is a natural candidate to study in PCOS.
Has the Wegovy pill been tested in women with PCOS?
No trial has yet tested the Wegovy pill in PCOS at its full weight-loss dose. But studies have looked at the use of semaglutide in women who have PCOS. The evidence on Wegovy and PCOS is promising but still in its early stages.
In one study of obese women with PCOS, almost 80% lost at least 5% of their body weight within three months. Their menstrual cycle also became more regular. Those women used the weekly semaglutide injection, not the tablet, and the sample size was small.
In another clinical trial, women with PCOS took semaglutide alongside metformin. Metformin is a common tablet for PCOS that helps lower insulin and blood sugar. The combination produced more weight loss, better insulin sensitivity, and regulation of periods than metformin alone. Those women, again, used the injection form of semaglutide rather than the tablet.
Early work on the oral form points the same way. In one trial, women with PCOS took either oral semaglutide or myoinositol alongside metformin. Myoinositol is a supplement often used for PCOS.
The semaglutide group lost more weight, had seen an increase in regularity of their periods, and showed a reduction in insulin resistance in comparison. These are early findings from a medical conference, not a full published study.
The weight loss that oral Wegovy produces is exactly the kind that helps PCOS. In the tablet’s own large trial, OASIS 4, the majority of participants were female. The 25 mg weight-loss dose has not had its own PCOS trial yet.

How much weight loss is needed to improve PCOS symptoms?
Losing around 5% of your body weight can noticeably improve PCOS symptoms. The NHS notes that a 5% drop in weight brings a real improvement in PCOS as it has shown to reduce insulin resistance and testosterone levels . For someone weighing 90 kg, that is roughly 4 to 5 kg. Losing a bit more can help further.
As the weight comes off, insulin drops and testosterone falls, so ovulation is more likely to return. That steadies the menstrual cycle and improves insulin sensitivity.
The results of Wegovy pill in trials clears that bar comfortably. In the OASIS 4 trial, adults on the 25 mg tablet lost around 13.6% of their body weight over 64 weeks. That compared with about 2% for those given a placebo, an inactive pill used for comparison.
The Wegovy pill results build over the first few months rather than all at once. For most, results are far more than the 5% needed to ease PCOS symptoms, and the benefits last well beyond the short term. Women with PCOS carry a higher long-term risk of type 2 diabetes — losing weight and keeping it off reduces the risk. This is why weight loss is the first thing we focus on for many women managing PCOS.
Does the Wegovy pill affect fertility in women with PCOS?
Weight loss can improve fertility in PCOS, and the Wegovy pill may help through this route. Regular ovulation is often the missing piece in PCOS. Losing weight can help the ovaries release an egg more regularly, which is why the tablet may help indirectly. The effect comes from the stabilisation of testosterone through improvement of insulin resistance, not through any direct action on the ovaries.
The Wegovy pill must not be taken during pregnancy. Semaglutide can remain in the body for weeks after stopping. The prescribing information advises stopping it at least 2 months before you try to conceive. If pregnancy is the goal, we can advise you on this.
One small study of semaglutide with metformin reported improved fertility in women with PCOS and obesity. Because of its limited size, the study can only hint at the benefits, not prove them.
Reliable contraception is important while you take the tablet, since the medicine is not safe in pregnancy. The Wegovy pill and the contraceptive pill have no interaction, and the contraceptive pill will still remain as effective while being on treatment.
Can I take the Wegovy pill with metformin?
Yes, the Wegovy pill and metformin can be taken in combination. Prescribers have used metformin to treat PCOS for years. The NHS recommends it to lower insulin and blood sugar and to regulate periods.
The two medicines work differently and work safely in tandem. The product information for the oral Wegovy tablet shows no meaningful change in how metformin behaves alongside it. You do not normally need to change your metformin dose. Be mindful of the Wegovy pill fasting rules when using medications together – the Wegovy pill requires at least eight hours of fasting before taking, and you should not take any food or other medications for at least half an hour after taking it.
In our experience, most women asking about the Wegovy pill for PCOS have already tried diet changes or metformin. They want to know what a GLP-1 medicine adds on top. For some, adding the tablet tackles insulin from a second angle and helps reduce resistance further. For others, metformin alone does the job.
What side effects should women with PCOS know about?
Most side effects of the Wegovy pill are mild and affect the stomach and gut. Feeling sick, loose stools, and constipation are the effects people notice most. They tend to peak when you start treatment or move up a dose, then ease as your body settles.
Constipation can outlast the early nausea for some people, so it helps to keep fluids up and increase your fibre intake. PCOS does not change this pattern. Most people find the early side effects fade within a few weeks.
When should I see a doctor?
Contact a healthcare professional if side effects turn severe or anything worries you. Speak to our clinical team about mild but ongoing effects. These include nausea, mild vomiting, mild diarrhoea, constipation, or reflux.
The team can also help with any questions about whether to stay on, pause, or step up your dose. Also inform us if your periods change after starting. A shift in your menstrual cycle can be a normal sign of better insulin resistance. We will need to discuss this with you to be certain.

Stop the Wegovy pill and call 999 or go to A&E for anything acute. Warning signs include severe stomach pain that spreads to your back. Having trouble breathing from a severe allergic reaction, signs of severe dehydration, or sudden loss of vision in one or both eyes.
