How do the results from oral Wegovy and injections compare?

Wegovy pills and injection are weight loss treatments that produce similar results in adults with obesity. In the OASIS 4 clinical trial, adults taking the 25mg Wegovy pill once daily lost around 13.6% of their body weight over 64 weeks. In the STEP 1 trial of the 2.4mg weekly injection, the average weight loss was 14.9% over 68 weeks. All participants also followed a reduced-calorie diet and increased physical activity.

Is the Wegovy pill as effective as Wegovy injections? For most patients, yes. The 1.3% gap between pill and injection is likely too small to matter in practice. Wegovy pill results varied widely in the trial.

Some people lost more than 15% of their body weight. Others plateaued earlier. In practice, individual results vary more widely than the average gap between pill and injection.

The pill and injection put about the same amount of the drug into the bloodstream once you reach the highest dose. When STEP 1 participants stopped taking the Wegovy injection, most put the weight back on. The same pattern likely applies to the pill over the long term, though data on regain rates has not yet been published.

A higher injection dose is also available. In the STEP UP trial, the 7.2mg weekly injection produced an average weight loss of 18.7% over 72 weeks. This dose is an option for people who do not reach their weight goals on 2.4mg.

How do the side effects compare?

Wegovy pill vs injection side effects largely overlap because both formulations deliver the same drug. The main side effects of the Wegovy pill differ from the injection in two main ways. Injection-site reactions only happen with the pen, and vomiting is slightly more common with the pill at the full dose.

Roughly half of adults on the daily pill experienced nausea at some point in the trial. The rate on the weekly injection was around the same. Almost a third of people on the pill experienced vomiting. On the injection, it was closer to a quarter.

Diarrhoea was more common on the injection (around a third) than the pill (around a fifth). Dyspepsia, which feels like heartburn or indigestion, went the other way: roughly 18% on the pill and 10% on the injection.

Skin sensitivities, or dysaesthesia, were slightly more common on the tablet. Symptoms include tingling, burning, or unusual pain on touch. Both OASIS 4 and the earlier OASIS 1 trial of oral semaglutide reported dysaesthesia. Dysaesthesia rates were notably higher with Wegovy 7.2mg injections. In the STEP UP trial, around 1 in 5 people on the higher dose reported skin sensitivity symptoms, compared with about 1 in 20 on the 2.4mg dose.

Most side effects hit during the early weeks of dose escalation. This is the hardest part of the routine and the most common reason for stopping treatment. Symptoms usually settle once you have spent a few weeks on the same dose, or once you reach the maintenance dose, which is why the schedule steps up gradually.

Around 1 in 20 people on the injection get mild redness or itching at the injection site. The pill does not cause this. Serious side effects were uncommon across trials for both formats.

What is the difference in how you take them?

The biggest difference between the Wegovy pill and injection is the daily dosing and fasting requirement. You should swallow one tablet whole with up to 120ml of plain water, on an empty stomach, first thing in the morning. Do not crush, split, or chew it.

Wait at least 30 minutes before you eat, drink, or take other tablets. Getting this right matters. Each time you break the 30-minute fasting window, less of the drug is absorbed. This is where most routines start to slip.

The Wegovy injection works on a weekly dosing schedule instead. You inject once a week into the stomach, thigh, or upper arm using a pre-filled pen.

The injection has no fasting rules and can be used at any time of day. Rotate the injection site each week to reduce the risk of skin irritation. Pick a day that works for you and stick to it.

How does the cost compare between Wegovy tablets and injections?

UK pricing for the Wegovy pill has not been set yet. A direct Wegovy pill vs injection cost comparison is not possible. The Wegovy pill vs injection cost gap will depend on UK launch pricing.

NovoCare Pharmacy is Novo Nordisk’s US direct-to-patient programme. The Wegovy pill is listed there at between $149 and $299 a month, depending on strength. Injectable Wegovy costs between $199 and $399 a month on the same programme. The full Wegovy list price in the US is around $1,349 a month, but most people pay less through insurance or savings programmes.

The pill does not need cold storage or costly pen devices, which should make it cheaper to produce. In the UK, whether the Wegovy pill is cheaper than the injection will depend on how Novo Nordisk prices the two formulations at launch.

Is Wegovy pills or injections better for weight loss?

Which is better, the Wegovy pill or injection? Neither format is objectively better for weight management. The Wegovy pill vs Wegovy injection choice is about routine, not effectiveness.

Both are GLP-1 medications that deliver semaglutide at similar levels in the body. The difference is where each routine becomes harder to maintain.

With the pill, the routine can slip before you notice. Cut the fasting window short, and less of the drug gets through. If your mornings vary, you rely on an early coffee, or you work unpredictable shifts, the pill might underdeliver. But for some people, avoiding needles makes the trade-off worth it.

With a weekly injection, it can be easier to put off. A daily pill builds consistency simply because you take it every morning.

What about the dose difference?

The Wegovy pill dose is much higher than the injection dose, but it is not stronger. The maintenance dose is 25mg for the daily pill, and 2.4mg for the weekly injection. Why the Wegovy pill dose is higher than the injection comes down to absorption. Less than 1% of the swallowed dose reaches the bloodstream, compared with roughly 89% from the injection.

The gut breaks down most of the tablet before it can be absorbed. This means a much higher dose is needed to deliver the same amount of drug as the injection. Once absorbed, the amount of drug in the bloodstream matches what the weekly injection provides. Similar blood levels explain the similar weight loss.
