Nomination Agreement

The NHS Electronic Prescription Service (EPS) in England allows you to nominate a pharmacy to electronically receive your prescription.

By using Simple Online Healthcare, you are authorising us to nominate our NHS registered pharmacy Davis Chemist to receive your prescriptions. These will be sent to us from your GP surgery, via EPS or other methods. This setting is stored on the NHS database (spine) to facilitate our receipt of your prescriptions. You may change or remove the pharmacy nomination at a later date.

You can nominate any pharmacy that uses EPS to receive your prescriptions, this does not have to be a pharmacy local to you. Simple Online Pharmacy offers a national NHS delivery service via Davis Chemist. You can only nominate one pharmacy at any one time, you can change this by speaking to your GP or notifying the interested pharmacy.

By nominating Simple Online Pharmacy for EPS, this will make any future orders simple, efficient and swift as a direct link is established between your GP surgery and our Davis Chemist. By agreeing to this, you consent to us contacting your GP surgery for any queries relating to your prescription items. 

EPS is secure, safe and confidential, your prescriptions will automatically be sent to us unless you request otherwise. You consent to your data being securely handled by Simple Online Healthcare.

If you do not want your prescription sent to a nominated pharmacy during a face to face consultation, you may ask your prescriber to issue the prescription as a Phase 4 non-nominated or paper prescription. This can physically be taken into any pharmacy of your choice.   

If there are any prescriptions which have not yet been dispensed for you or have previously been sent back to the spine, then the change of nomination will allow for these to also be dispensed and delivered to you. This takes all prescriptions into consideration, which includes acute prescriptions also. Your NHS prescriptions which are sent electronically to Davis Chemist where they are claimed will be dispensed and shipped directly to you.

Due to time constraints, the delivery service may not be suitable for urgent prescriptions using this model.
