NHS Summary Care Record
This only applies to patients registered in England. If you are registered with a GP surgery in England, your summary care records are automatically generated and protected by secure technology unless you have opted out of this. By giving Simple Online Healthcare authorisation to access your summary care record, you will allow our NHS pharmacy, Davis Chemist, to view your data if required.
As well as your name, address, date of birth and unique NHS number, your summary care record will contain information relating to your prescribed medication. Information contained within your summary care record will also include allergies and any adverse reactions to any medication that you have had.
Access to your summary care record, should it be needed, will enable us to have a clearer understanding of your requirements when we are dispensing your medication. Our pharmacists may want to double-check that the medication you have been prescribed is correct and appropriate. As we are committed to providing you with safe and effective care, this information will help to safeguard you of potential errors which will enhance your patient experience. The people who may access your NHS summary care records are our regulated and authorised healthcare professionals.
If your summary care record is accessed, the relevant details including the time, date and name of the healthcare professional will be recorded should you wish to later make a subject access request to NHS digital. If you would like to change the permission you give us, please let us know in writing. If you would like to opt-out of the summary care record please contact your GP surgery, or fill out the following form: https://digital.nhs.uk/services/summary-care-records-scr/scr-patient-consent-preference-form
